This patient has already undergone elastic MACS and neck lifting. Today, we are carrying out elastic lifting of the eyebrows together with lifting of the upper lip.

An upper lip that is too long is a sign of old age and needs to be corrected. Indeed, like all the soft tissues of the face, the lip is also subjected to gravitational ptosis. The traditional procedure performed to shorten the white portion of the upper lip leaves a scar at the base of the columella and the nostrils. This scarring is not acceptable to all patients.

This patient had an acute nasolabial angle, which was corrected. Six months later, the inclination of the nose appeared optimal, but the patient still presented dysmorphism caused by the injection of liquid silicone into the vermilion border. The silicone has caused the vermilion border to protrude too far in relation to the lip.

27. Correction of the naso-labial angle with the elastic thread and two-tipped needle

In women, the naso-labial angle should be greater than 90°. An acute angle is less feminine and makes the nose look longer. The naso-labial angle can easily be corrected by means of an EP4 elastic thread mounted on a two-tipped needle.
Local anaesthesia is carried out by means of mepivacaine with epinephrine. A N° 11 blade is used to make a skin incision a few millimetres long at the angle formed between the anterior portion of the right wing of the nose and the lip. Once the skin has been incised, a Klemmer is used to create a deep pocket that reaches the bottom of the bony fossa present in this region.
