The lower eyelids of this young patient present extensive xanthelasmas. Timedsurgical resurfacing is the technique of choice for eliminating xanthelasmas, even those which recur. What you see here is one of the more than 72 techniques of Timedsurgery. Local anaesthesia is carried out by means of mepivacaine with epinephrine. The xanthelasmas will be eliminated by means of timedsurgical resurfacing, which does not heat the deep tissues. Treatment is normally carried out in two sessions. During the second session, which is held two months after the first, a magnifying glass is used to check whether any residual areas of xanthelasma are still visible. If they are, they are treated by means of the same program data used in the first session. The Timed apparatus is set to the resurfacing function in the coagulation mode and the power is set to 38 or 50 W. An EM 15 electromaniple is mounted. The tip of the electromaniple, which has a diameter of 1.5 mm, must be placed very close to the surface of the skin in order to generate the micro-arcs that vaporise and dry the lesion. The electromaniple must be kept moving at all times. The operator carries out several sweeps in the areas where the xanthelasma is thickest. Resurfacing must also be carried out a few mm around the visible lesion.

Areas of stable vitiligo and piebaldism can easily be repigmented by means of timedsurgical de-epithelialisation followed by the grafting of autologous keratinocytes and melanocytes cultivated from a 1 cm² fragment of skin taken from a normopigmented area about 20 days earlier. Following a local anaesthetic, the skin of the lip is de-epithelialized using a specific Timed-surgical programme, which was created for repigmentation of the stable vitiligo, powered at 2 or 3 watts.
